St. John's wort ( Hypericum perforatum ) is one of the most widely marketed extracts in the world. Its popularity and fame date back to the times of the ancient Greeks and Romans, and its use has continued over time, starting in the Middle Ages and continuing to the present day, where extracts from this plant are widely used in phytotherapy for relaxation and mental well-being.

St. John's Wort is a perennial herbaceous plant native to the British Isles and now considered ubiquitous, given its worldwide distribution. Its full botanical name: It's Hypericum perforatum , a name derived from its leaves, which when held up to the light appear as if they have holes, that is, as if they were perforated. In reality, these aren't holes, but rather small oily blisters. St. John's wort is, in fact, "St. John's wort," but also "devil-chasing" herb, because in the past it was believed that sleeping near St. John's wort plants kept evil spirits away and protected from death for at least a year. The term "St. John's wort" likely derives from St. John's wort's flowering period, which occurs in late June (therefore coinciding with the celebration of St. John's Day, June 24th). This plant grows in sparse, sunny woods and is cold-resistant. In some areas of the world, it is considered a weed, while in Europe it is cultivated primarily for its effects on relaxation and mental well-being.

The properties of St. John's Wort's Wort flowering tops, of which the fresh buds and flowers are used, are due to the phytocomplex it contains, consisting essentially of flavonoids such as hypericin, rutin, quercetin, and hyperoside, along with other substances that contribute to relaxation and mental well-being, which are obtained from the dry extract or mother tincture. The anxiolytic effect of this plant is exerted through the activation of the benzodiazepine receptor, although the specific molecule responsible for this activity has not been identified. Studies show that the compounds that make up the extract, when administered alone, do not have the same efficacy as the extract as a whole, confirming that the anxiolytic effect is indeed due to the combination of the various molecules that compose it.

The same synergistic action of the compounds also underlies the antidepressant effect of this plant. The effect is exerted in this case by inhibiting norepinephrine reuptake, inhibiting certain serotonin and dopamine receptors, and inhibiting GABA reuptake at the presynaptic level, and is due to the combination of hyperforin, hypericin, and rutin.

Due to these characteristics, St. John's Wort extract is used to treat anxiety and minor depressive disorders.

Extensive pharmacological and clinical studies conducted on the dry extract of St. John's wort demonstrate that it is a safe herbal remedy, but it may present some contraindications that should definitely be considered. St. John's wort interferes with the metabolism of numerous drugs, enhancing or, in some cases, nullifying their effects. Therefore, it is not recommended to use it concurrently with other therapies, as this may stimulate certain drug-metabolizing enzymes, resulting in a reduction in circulating levels. Furthermore, its effects are additive with those of serotonergic drugs. Clinical studies have shown that taking this medicinal herb reduces blood levels of drugs such as digoxin (a substance belonging to the digitalis glycoside class, used as a medicine for certain heart conditions) and theophylline (an active ingredient used to increase the force of cardiac contraction), and may also reduce the effect of warfarin (the most common anticoagulant). It can also lower blood levels of cyclosporine, an antibiotic used to treat transplant rejection, and reduce the effectiveness of oral contraceptives. Additional attention should also be paid to photosensitivity during exposure to sunlight. For this reason, it is advisable to avoid sun exposure and/or tanning lamps during the period of use, especially if you have sensitive and delicate skin.
